Oculoplastics — Advanced Eyelid Protocol

Blepharoplasty | PPA — Advanced Eyelid Protocol

Structured eyelid assessment in São José do Rio Preto – SP, focused on eyelid function, the balance of the gaze and an individualised plan for each case.

PPA is a clinical assessment and planning protocol, not a recognised medical specialty. Any surgical indication depends on an in-person examination and varies from person to person.

  • Eyelid assessment

The consultation starts with an examination of the eyelids and periocular region: upper and lower lid position, tone, excess skin, crease height, symmetry and the relationship with the eyebrow. The ocular surface and tear film are also assessed.

  • Function before aesthetics

When excess skin or eyelid drooping interferes with the visual field, the first goal is functional. Reading function and appearance together avoids unnecessary indications and guides technique selection.

  • Natural appearance

Planning aims to preserve each person's expression. Conservative measurements and respect for individual anatomy are preferred over broad changes to the shape of the gaze.

  • Individualised planning

Age, anatomy, dry eye history, general medical conditions and expectations are discussed before any decision. The plan is presented with its alternatives, including the option of not operating.

  • Upper blepharoplasty

Treatment of excess upper eyelid skin, considering the visual field, the eyelid crease and symmetry between both sides.

  • Lower blepharoplasty

Indicated in selected cases, assessing fat pads, the tear trough, lower lid tone and the risk of retraction.

  • Associated procedures

Correction of ptosis, entropion, ectropion or lacrimal pathway changes is performed only when clinically indicated, never as a standard package.

Recovery

  • Swelling and bruising are expected in the first days and subside gradually.
  • Written instructions on compresses, local hygiene and eye drops are provided.
  • Intense physical activity and sun exposure are restricted for a period defined in consultation.
  • Follow-up happens at scheduled visits, with intervals defined case by case.

Limitations and responsibility

  • No result can be guaranteed: healing is individual.
  • Pre-existing asymmetries may partly remain after the procedure.
  • Dry eye can be temporarily accentuated and must be assessed before surgery.
  • Every surgery carries risks, discussed in consultation before consent.

Frequently asked questions

What is PPA?

It is the eyelid assessment protocol used in the practice: a structured examination, standardised photographic records when indicated, and individualised planning. It is neither a medical specialty nor a guarantee of results.

Is blepharoplasty always aesthetic?

No. Many cases have a functional component, when excess skin or lid drooping reduces the visual field. The assessment defines the nature of each case.

How do I know whether it is indicated for me?

Only through an in-person examination. Photographs and reports help the initial conversation but do not replace clinical assessment.

How long does recovery take?

It varies with the technique and the person. The first days concentrate swelling and bruising; return-to-activity timings are advised individually.
